Registering with the surgery

To register with the surgery:

fill out an online registration form
visit the surgery to collect a paper registration form. Monday to Friday from 8am to 6.30pm.

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

Please complete a registration form for each member of the household wanting to register.
