PATIENT ACCESS RE-SIGN UP

How to Re-Sign up to Patient Access

  1. If you are a Toft Road or Manchester Road patient and you are struggling to get back on to patient access please can you first of all contact your surgery for a new code/linkage key.
  2. First of all you need to Sign in to Patient Access using the details you have always used (this needs to be the email address and not account ID)
  3. Select your name in the top right-hand corner, then ‘My Account’.
  4. Scroll down on the Account settings screen, then select ‘Link to practice’.
  5. Enter the postcode – WA16 8HR.
  6. Select the surgery from the search results, this may come up as Knutsford Medical Partnership or Annandale Medical Centre. (DO NOT CLICK MANCHESTER ROAD OR TOFT ROAD)
  7. It will then ask if you have received a registration letter from your practice, if you have received your linkage key/code you need to select YES.
  8. Then follow the next steps to complete.
